FOCUS OF THE MONTH

The ICMS Info Center Has Moved!

We’re excited to announce the launch of the new ICMS Information Center on our main HSH site—an improved continuation of the Info Center/System of Care Hub.


Content from the original Info Center will begin migrating to the new hub (starting with PH ICMS, with more program areas to follow). The new platform includes all the familiar resources—now in a more modern, flexible format.


As a refresher, the Info Center features:

  • Program-Specific Dashboards (coming soon)
  • Featured/New Content
  • Guides & Tip Sheets
  • Case management forms and links
  • Latest The Focus
  • Meetings, trainings, and office hours info
  • Glossaries and more

Spotlight Update:

We're absolutely thrilled with the number of nominations we're already received for Spotlight features! We have created a photo release consent form that will require completion prior to Spotlight features. We will begin reaching out to nominators directly to begin the process of completing any required documentation.

ICMS Snapshot Updates

Most Recent HMIS Assessment Added to Participant Level Summary


Four new indicators were added to the Service Delivery and Documentation Tracking category of the Participant Level Summary tab.


The additional indicators include:


  • Most Recent HMIS Universal Data Elements Assessment – Indicates the most recent date an HMIS Universal Data Elements assessment was completed in CHAMP. This assessment includes core participant information required for HMIS reporting, such as demographics, housing history, and project enrollment details, and serves as the foundation for system-level reporting and analysis.


  • Most Recent HMIS DV Assessment – Indicates the most recent date a Domestic Violence (DV) assessment was completed in CHAMP. This assessment captures whether a participant is experiencing or has experienced domestic violence, supporting appropriate service coordination, safety planning, and system-level understanding of needs among survivors.


  • Most Recent HMIS Financial Assessment – Indicates the most recent date a Financial assessment was completed in CHAMP. This assessment includes information on income and non-cash benefits, supporting evaluation of financial stability, access to resources, and changes in income over time.


  • Most Recent HMIS Barriers Assessment – Indicates the most recent date a Barriers assessment was completed in CHAMP. This assessment captures participant-reported conditions that may impact housing stability and service needs, such as chronic health conditions, mental health, substance use, disabilities, and HIV/AIDS status.
