FOCUS OF THE MONTH

The ICMS Info Center Has Moved!

We’re excited to announce the launch of the new ICMS Information Center on our main HSH site—an improved continuation of the Info Center/System of Care Hub.


Content from the original Info Center will begin migrating to the new hub (starting with PH ICMS, with more program areas to follow). The new platform includes all the familiar resources—now in a more modern, flexible format.


As a refresher, the Info Center features:

Spotlight Update:

We're absolutely thrilled with the number of nominations we're already received for Spotlight features! We have created a photo release consent form that will require completion prior to Spotlight features. We will begin reaching out to nominators directly to begin the process of completing any required documentation.

ICMS Snapshot Updates

March 31st, 2026

Four new indicators were added to the Service Delivery and Documentation Tracking category across the Provider Level, Building Level, Scattered Site Level, and Case Manager Level Summary tabs.

The additional indicators include:

  • Demographic Profile Updated Within the Past Year – Displayed as a percentage. Represents the proportion of active cases with a Demographic Profile update within the past year.
  • Household Composition Profile Updated Within the Past Year - Displayed as a percentage. Represents the proportion of active cases with a Household Composition Profile update within the past year.
  • Medical Profile Updated Within the Past Year - Displayed as a percentage. Represents the proportion of active cases with a Medical Profile update within the past year.
  • Legal Profile Updated Within the Past Year - Displayed as a percentage. Represents the proportion of active cases with a Legal Profile update within the past year.


March 27th, 2026

Two new indicators were added to the Service Delivery and Documentation Tracking category in the Participant Level Summary tab.

The additional indicators added include:

  • Most Recent Medical Profile Update – Indicates the most recent occurrence of the “Save” button being clicked on the Medical Profile page in an ICMS participant’s CHAMP profile (includes self-reported physical and behavioral health information, mobility and accessibility needs, medical and mental health conditions, and ADLs/IADLs). Clicking the “Save” button represents the case manager’s attestation that the participant’s Medical Profile information has been reviewed, updated as needed, and confirmed.
  • Most Recent Legal Profile Update – Indicates the most recent occurrence of the “Save” button being clicked on the Legal Profile page in an ICMS participant’s CHAMP profile (includes self-reported conviction history and other legal challenges). Clicking the “Save” button represents the case manager’s attestation that the participant’s Legal Profile information has been reviewed, updated as needed, and confirmed.

It is expected that each of these profile sections is reviewed and updated or confirmed at least once annually, and whenever significant changes occur that should be reflected in the participant’s CHAMP record. If the most recent save action occurred more than one year ago, or reflects only the initial creation of a skeleton participant profile to initiate enrollment, the cell will be highlighted in salmon to indicate that an update review is needed.

 

A new indicator was added to the Current Resources and Statuses category in the Participant Level Summary tab.

The additional indicator added includes:

  • Universal Housing Application (UHA) Status – Indicates the most recent Universal Housing Application (UHA) status on record for the participant, when applicable. For participants who are not yet permanently housed, or who are permanently housed and have a subsidy exchange or relocation request in process, this indicator provides a brief summary of the most recent UHA step completed. This information can be used to determine whether further review is needed in the UHA Log and to support targeted housing navigation efforts.

 

The UHA Log tab was added to the ICMS Snapshot.

This tab provides a list of all Universal Housing Applications (UHAs) on record for active ICMS participants. Each row represents a unique UHA. For each UHA, the tab includes housing resource details, as well as statuses, dates, and notes for each step in the UHA process. The UHA Point of Contact (POC) on record is also included. This information can be used to support targeted housing navigation efforts and timely follow-up on outstanding UHA steps.

Clicking the “Save” button represents the case manager’s attestation that the participant’s household composition information has been reviewed, updated as needed, and confirmed.

It is expected that this information is reviewed and updated or confirmed at least once annually, and whenever significant changes occur that should be reflected in the participant’s CHAMP record.
