Notification of Privacy Practices


The Notice of Privacy Practices describes the DHS pledge to keep each participant’s health information private and secure, the different ways the HFH system of care (including ICMS practitioners) may use, disclose, or share a participant’s health information, and participant rights regarding their health information.


The Notice of Privacy Practices should be discussed with and provided to each ICMS participant right before the Universal Sharing Consent is reviewed and signed. Upon providing the Notice of Privacy Practices to the ICMS participant, the ICMS practitioner is expected to have the participant sign the Acknowledgement of Receipt of the Notice of Privacy Practices Form.

The Acknowledgment of Receipt of the Notification of Privacy Practices is expected to be completed even if no signature is obtained by the ICMS participant.

ICMS Snapshot Updates

The Notification of Privacy Practices on Record? indicator highlights which active ICMS enrollments in CHAMP have record of an ICMS participant receiving an overview of DHS Privacy Practices. A "Yes" indicates there is a Notification of Privacy Practices on record and that another one is not needed. "Declined" indicates that the participant declined to sign the Acknowledgement of the Notice of Privacy Practices form the last time it was reviewed with them. It is advised for ICMS practitioners to periodically check back in with participants who previously declined to acknowledge DHS Privacy Practices, in case the participant changes their mind. When this data point is blank, it indicates there is no record in CHAMP that DHS Privacy Practices have been reviewed with the ICMS participant, and it is advised that the ICMS practitioner reviews this information with the ICMS participant ASAP.
