FOCUS OF THE MONTH

Connecting to Caregiving

If you work with participants who have unmet ADL/IADL needs, caregiving resources funded through the State of California and LA County are available to help.

Caregiving helps participants with co-occurring conditions and/or disabilities remain in their homes or in transitional shelter settings instead of moving to a nursing home or board and care facility.

Whether a participant needs accompaniment to appointments, support with daily tasks such as bathing and cleaning, or even more extensive support such as hospice care, there are caregiving resources within reach.

California’s In-Home Supportive Services (IHSS) program funds ongoing caregiving support and is administered by the Department of Public Social Services (DPSS) in LA County.

Once an IHSS application has been submitted, ICMS can request interim caregiving support through HFH's In-Home Caregiving (IHCG) program.

IHCG serves as a temporary bridge that offers quicker access to caregiving support until an IHSS application is approved.

To learn more about helping ICMS participants connect to and navigate caregiving resources, as well as how to perform care plan action steps that ensure the greatest level of connection to all caregiving resources, access HFH's Caregiving Information Center & Cheat Sheet :
