We’re excited to announce that the new and improved CHAMP Cheat Sheet is now available!
On the front side of the sheet, you’ll find a newly revised layout that includes updated information and helpful reminders on subjects including consents, assessments, outreach, PH Updates, the ICMS Snapshot, and more!
On the back side of the sheet, we refreshed the Service Options & Action Steps table to include all ten eligible PSH ICMS Service options in CHAMP.
Service option definitions have also been added to provide better clarity on when each option is most appropriate. To the right of each Service option, you'll find a list of Action Steps commonly taken by case managers that support recording that Service option.
