Steps to Permanent Housing

Exiting homelessness, particularly long-term homelessness, is a journey

The process of exiting homelessness, particularly long-term homelessness, is more than complex: for some, it can be impossible without experienced support and guidance. Internal struggles, like mental health issues, substance misuse disorder, or physical and emotional trauma, coupled with external barriers, such as application fees, complicated applications and forms, and stereotyping and biases, can all stall or even derail a person's journey out of homelessness.

Our case management team works hard every day to meet clients where they are, wherever they are, on their journey out of homelessness. Each client and case manager team works through the steps to permanent housing, which can take weeks, months, or even years, depending on the individual and their needs.

Read through our most recent Annual Report to learn more about each of the steps to permanent housing, and hear about some of the progress CCEH has made in our mission to end homelessness in the greater Concord community.

Our narratives about ending homelessness are backfiring. Simply rehashing the problem does more harm than good. Instead, state and local leaders must help citizens see how solutions to homelessness benefit all of us.

For NYC's most vulnerable residents - some thirteen thousand people with significant mental- or behavioral-health diagnoses, who regularly spend the night at a shelter, in the subway, or on the street - one of the biggest challenges can be getting from one point of care to the next.
