Health e Paradigm Active Around Georgia
Health e Paradigm is active in Georgia. We continue to focus efforts on engaging physician practices and hospitals throughout the state regarding the importance of health information exchange participation and how it can benefit those seeking to improve patient outcomes while transitioning to the new payment models. We recently offered an educational luncheon at the Compass Practice Transformation Network in Augusta, GA to encourage quality teams to use health care data in the most meaningful way to improve quality of care.

As I travel the state and meet with physicians, administrators, and IT staff, I receive great questions I'd like to share one here:

Q: How are the Health e Paradigm services and analytics reports different than what my EHR already provides?
A: My answer is always the same. The benefits of using Health e Paradigm far exceed the capabilities of an EHR. Realistically, physicians don't use the same EHR system. If you are only sharing patient data with clinicians within your health system, you are not accessing patient information from all of the other providers, your patients see. Physicians can provide better patient care when they have access to "all" patient data coming from all participating health information exchange (HIE) physicians. The Health e Paradigm HIE presents providers with a more complete picture of the care each patient is receiving and the longitudinal data is aggregated in a way that presents opportunities for you to be more successful in quality reporting. These analytics tools have been uniquely developed to assist in meeting the requirements for MIPS/APM. (See " Questions To Ask When Choosing an HIE " below to learn more.)

The Health e Paradigm team remains committed to increasing awareness of the MIPS/APM requirements that physicians who accept Medicare must now meet and to explaining how this statewide effort can ease the transition to the new payment model while favorably impacting daily patient encounters. The next educational program will be held at Pebble Hill in Thomasville, GA. Please check the Health e Paradigm website frequently to learn of upcoming education programs in your area.
Questions To Ask When Choosing an HIE
Health e Paradigm is positioned to transform health care. With Health e Paradigm, Georgia's physicians and health care providers can participate in a health information network that comprises best-in-class technology partners, is provider-led, financially secure, innovation-driven and responsive to the quickly changing landscape of health care that mandates timely access to relevant products and services.

We know that Health e Paradigm is unique and the only option in Georgia that integrates ALL the features listed below into one platform. As you evaluate HIE options we encourage you to ask these questions:

  1. Will the exchange be integrated into your EHR?
  2. Will it include a personal health record across all of your hospitals and clinics?
  3. Will it be provider-led and ensure you have the ability to control who has access to your data?
  4. Will it help your clinic providers monitor and report their quality measures for Merit-Based Incentive Payment System or an Advanced Payment Model?
  5. Is it a Qualified Clinical Data Registry (QCDR) for re-opting to CMS?
  6. Does it provide alerts to your affiliated doctors and others, notifying caregivers the patient has been admitted into your hospital?
  7. Does it include population health analytics that can help you identify health disparities in your community and complete your community health assessment?
  8. Does it provide hot spotting for high Emergency Department utilizers?
  9. Does it assist you in identifying Medicare readmissions that result in penalties for your hospital?
  10. Does it identify other locations where your patient has readmitted within 30 days?
  11. Does it help you with referrals from community doctors?
  12. Does it allow community doctors to see the information from your hospital within their EHR?
  13. Does it provide HEDIS data to your payers automatically?
  14. Does it include an ONC certified personal health record that provides access to high quality health and health education information?
  15. Does it include messaging, allowing you to communicate with your patients securely?
  16. Are you able to request ad-hoc de-identified analyses that meet your needs?

The technology platform that supports Health e Paradigm is being successfully deployed through state medical societies in Kansas, Connecticut, South Carolina, New Jersey and Missouri. MAG is proud of its role in shaping this suite of products and services, and to have been the first medical society to partner with KaMMCO Health Solutions (KHS).
