Northeast Health Partners (NHP) is the Regional Accountable Entity (RAE) responsible for improving the health and wellness of our community members in the Northeast region of the state (Region 2). Our mission is to provide a comprehensive health plan that integrates physical and behavioral health care in a way that improves the the quality of life for members by ensuring they are connected to health care supports every step of the way. We proudly serve the following counties: Cheyenne, Kit Carson, Lincoln, Logan, Morgan, Phillips, Sedgwick, Washington, Weld & Yuma.

Program Spotlight: Weld CDPHE Heart of Weld Program


Heart of Weld, provided by Weld CDPHE, is part of the National Diabetes Prevention Program, led by the Centers for Disease Control and Prevention (CDC). It is proven to help people with prediabetes prevent or delay development of type 2 diabetes. Through the Heart of Weld group, individuals work with other participants and a trained lifestyle coach to learn the skills necessary to make lasting changes. Heart of Weld groups meet for a year - weekly for the first 6 months, then once or twice a month for the second 6 months to maintain healthy lifestyle changes. During each session, the lifestyle coach will teach a lesson and lead a group discussion.


Classes are free of charge, offered in English and Spanish and can be attended in-person or virtually. Click HERE for the upcoming class schedule.

Throughout her career, she has blended her passion for helping others with her interest in data-driven solutions. For as long as she can remember, Diamila has been committed to supporting those in need and making a meaningful impact. Her education has provided her with the skills to enhance healthcare systems and improve outcomes for communities. She is dedicated to using her expertise to create positive change and empower others to thrive.
