Northeast Health Partners (NHP) is the Regional Accountable Entity (RAE) responsible for improving the health and wellness of our community members in the Northeast region of the state (Region 2). Our mission is to provide a comprehensive health plan that integrates physical and behavioral health care in a way that improves the the quality of life for members by ensuring they are connected to health care supports every step of the way. We proudly serve the following counties: Cheyenne, Kit Carson, Lincoln, Logan, Morgan, Phillips, Sedgwick, Washington, Weld & Yuma.

Provider Corner: Let's Talk Contracting with NHP in ACC 3.0

NHP has been selected by the Department of Health Care Policy and Financing (HCPF) to continue as the RAE for Region 2, with expanded counties, under the new Accountable Care Collaborative (ACC) Phase III RAE contract, effective July 1st, 2025. More information is available HERE.

We are committed to ensuring all providers have the latest information on contracting with NHP.

Behavioral Health Providers: We are transitioning the management of Behavioral Health provider contracting and network management to Rocky Mountain Health Plans (RMHP) starting July 1, 2025. For more details and next steps, please refer to our BH Provider Letter.

PCMPs: NHP will be handling PCMP contracting and network management. For additional information and next steps, please refer to our PCMP Contracting Letter.

Interested In eConsult?

Having trouble finding available specialists to refer your Medicaid members to? Safety Net Connect can help connect PCPs to a network of specialty providers to enhance patient care and outcomes. The number of specialty providers continues to increase, giving PCPs options for caring for complex patients. AND, the good news is, there is still time to participate in NHP’s eConsult Platform Incentive program. Earn up to $10,800 per practice site for enrolling, training providers, IT connection support and completion of a consult.

NHP Staff Highlight: Meet Our Provider Network Consultant

Alma Mejorado

Alma is joining the NHP team after taking a sabbatical abroad – a long-anticipated personal goal. She moved to Colorado in 2017 from her native Southern California where she obtained her master's degree in Social Work. She applied her degree in the healthcare industry first as a grant writer then provider relations, before progressing into contracting and network development. Alma has worked with NHP since its foundation in 2018 and is now delighted to be an official member of the NHP team. Alma is passionate about the Colorado outdoors and spends her free time running, hiking and camping.

Fun Fact: Alma has encountered six bears during her trail runs and races in the Colorado mountains. They have been at a safe distance except the encounter with a Momma bear and her 2 cubs that led to the group backtracking for more than a mile.
