Northeast Health Partners (NHP) is the Regional Accountable Entity (RAE) responsible for improving the health and wellness of our community members in the Northeast region of the state (Region 2). Our mission is to provide a comprehensive health plan that integrates physical and behavioral health care in a way that improves the the quality of life for members by ensuring they are connected to health care supports every step of the way. We proudly serve the following counties: Cheyenne, Kit Carson, Lincoln, Logan, Morgan, Phillips, Sedgwick, Washington, Weld & Yuma.

Apply For Health First Colorado (Medicaid) or Check the Status of Your Coverage

If you’re in Colorado and need to check your Medicaid status or apply for coverage, there are several convenient ways to get started:



  • Visit the PEAK Website: Head to PEAK to apply or check your eligibility. PEAK is the Colorado state portal for Health First Colorado and other assistance programs.
  • Download the PEAK App: Apply and check your status on the go! The PEAK app is available for download on iOS and Android devices for easy access.
  • Call for Assistance: If you need help or have questions, the Health First Colorado Member Contact Center is available at 1-800-221-3943.
  • Visit Your Local County Office: Prefer in-person assistance? You can apply or inquire about your Medicaid status by visiting your local County Human Services office.

This healthy child clinic will be for those 6-months to 5 years of age, and will include free screenings for vision, hearing, speech, dental, height, weight, development and more.

Joanna Martinson

Joanna is a proud descendant of some of the first settlers in northern Colorado and continues to live on her family farm, where her husband, Terry, manages the operations. Her deep rural roots fuel her passion for supporting rural communities. Joanna is the mother of three daughters, and a grandmother to one beloved grandson. Since becoming a nurse in 1981, Joanna has remained energized by her work, finding fulfillment in collaborating with exceptional teams. Specializing in behavioral health, trauma-based care, and care coordination, she is committed to a person-centered, holistic approach to improving lives. Throughout her career, Joanna has worked in schools, public health, psychiatric hospitals, and children’s residential programs, and has also taught parenting courses. An avid traveler with a curious spirit, Joanna embraces adventure and continuously seeks new experiences. She leads with the belief of Dr. Martin Luther King Jr.: "The time is always right to do what is right," striving to make a meaningful difference in the lives of those she serves.


Fun Fact: Joanna Played on the first Colorado State Volleyball team, which was the first Champion girls team!
