The Weld Trust awards health and education related grants to nonprofits, schools and government entities located in Weld County or exclusively for Weld County residents. Our vision is to help Weld County become the healthiest and most educated county in the state of Colorado.

Through Proactive Grants, The Weld Trust identifies and addresses significant community needs by bringing the right partners together to address them. This grantmaking approach supports collaboration among nonprofit organizations, schools and government partners to fund shared solutions that advance health and education in Weld County. Unlike traditional responsive grants, where organizations apply for funding and applications are reviewed for approval, Proactive Grants begin with The Weld Trust aligning resources to support collaborative solutions.



Below are a few highlights of recent Proactive grants.

Access to behavioral health services in Weld County is expanding through the new Public Health Mobile Unit.


The Weld Trust funded a grant that brought together North Range Behavioral Health and the Weld County Department of Public Health and Environment to increase access to care across the county.



The Mobile Unit will operate 40 hours per week throughout Weld County and provide services such as behavioral health consultation, early intervention, psychoeducation, family support, referrals, community resource navigation, and crisis management.

At the Fort Lupton High School graduation ceremony in May, the salutatorian thanked an Inspire counselor for reminding her of her incredible potential and helping to secure financial aid to attend Colorado State University.

The Inspire program works alongside high schools to support seniors through leadership opportunities, college information, character development, career and college counseling, financial aid workshops, campus visits, mentoring and leadership development.

“It is having a profound impact here,” Weld County RE-8 Superintendent Dr. Justin Ralston said. “Having Inspire on the menu of options for students I huge. We can start to see changes academically and in our graduation rates.”

The Weld Trust awarded the district a two-year grant to support its partnership with Inspire. The investment reflects The Weld Trust’s commitment to supporting programs that strengthen education, expand opportunities and help students build a path toward long-term future success.

Colorado Youth Sports Giving Day kicks off September 1 with a 15-day giving period to help nonprofits raise funds that expand access to youth sports. Participating organizations will also have opportunities to stretch donation dollars through matching funds.

The Weld Trust will provide matching funds for eligible Weld County nonprofits participating in Colorado Youth Sports Giving Day through a Proactive grant.

Coal Creek Meals on Wheels, based in Lafayette, received a two-year Proactive grant to expand its home-delivered meals program into Frederick, Firestone, and Dacono. These rapidly growing Weld County communities continue to experience gaps in access to nutritious meals. In 2023, approximately one in eight individuals in the region experienced food insecurity.

The organization also conducts in-home assessments for new participants to better understand each person’s nutritional needs, health conditions, mobility challenges and overall living environment. This deeper level of engagement allows Coal Creek Meals on Wheels to provide more tailored meal options and connect participants with other community resources, such as transportation, healthcare navigation and housing support.



The expansion aims to increase access to nutritious food, improve health outcomes and help participants maintain independence in their homes.
