Dear Parents/Guardians,
Connecticut State Law and Middletown Schools require that all students entering Grade 7 receive a health assessment and updated immunizations prior to entry.
The health assessment includes completion of a physical examination and current documentation of any known medical issues that may impact your child at school, such as: asthma, diabetes, sickle cell anemia, mental health issues, or other known physical or mental conditions. The information is made available to the school nurse on the State of CT, blue Health Assessment Record (HAR).
Please be sure the following items on the HAR are complete and updated:
Part I: Parent contact information, health insurance carrier, emergency contact information, and parents’ understanding of a child's health history.
Please complete Part I prior to the physical exam appointment and discuss/confirm any issues of concern you document on the HAR with your child’s health care provider at the time of their physical examination. Be sure to read and sign the bottom of the form.
Part II: Health Care Provider documentation of physical examination findings and important information confirming the parent section (Part I). Any box on the HAR marked with an asterisk (*) must be addressed by the health care provider.
Immunization record: there is a list of required school–entry immunizations on the second page of the HAR. Be sure the health care provider has reviewed and updated any needed vaccines at the time of the physical exam or scheduled and documented on the HAR a follow-up date for an immunization appointment.
Physicals dated on or after June 1, 2021 of the grade 6 school year will be accepted as valid HARs for entry into 7th grade.
