GOLDEN DEMOBONE AWARD

NEW PRODUCT LAUNCHES
Medartis

Medartis and the recently acquired Nextremity team are preparing their first launches in Foot and Ankle. Nextremity, previously exclusively distributed by Zimmer Biomet, had to leave behind their commercially available products with ZB. However, their product pipeline and IP all went to Medartis in the sale and we are getting to see some of that technology come alive starting. Medartis is launching 2 products in the hot rotational bunion market.

The Medartis Lapidus Cut Guide is designed to provide anatomically-intuitive resection of the first tarsometatarsal joint in preparation for a Lapidus procedure. The Lapidus Cut Guide results in less resection of the bone, which can reduce the risk of shortening the first metatarsal. The design of the Lapidus Cut Guide also provides surgeons with a single instrument that allows for unconstrained angle correction based on the individual patient anatomy.

This cut guide comes sterile, is one time use, and is implant agnostic. The cost allows it to be used in ASC’s and can be combined with lower cost implants to maintain profitability of the case, which is the name of the game these days. It makes sense from a cost perspective versus some of the more expensive jig and implant products currently dominating the market today. 

This product is now commercially available.



The LapiPrep Lapidus Correction and Preparation Guide is an instrument system that provides surgeons with a comprehensive versatile jig and integrated modular components. The system is designed to allow for accurate and repeatable deformity correction and precise preparation of the first metatarsal and medial cuneiform of a patient’s foot to perform a Lapidus bunion procedure. Precisely controlling the IM angle, frontal plane rotation, and sagittal plane correction of the metatarsal, all while allowing surgeons the ability to distract and compress as needed is the hallmark of the system. The system design and modularity allows for curettage, saw cuts, or burr preparation as determined by the surgeon and their specific surgical preference, whether traditional open or MIS.

Medartis also wants to play in the jig space as well! The LapiPrep system is in pre-market trial, but is a comprehensive jig allowing for distraction and compression. It does not appear specific fixation has been created for the system, but you can be sure it is coming. Another player in the rotational bunion space coming after those Treace dollars.

Paragon28

Paragon28 strikes again and adds to their extensive Charcot System with some novel Central Column plates. While these will not be an everyday product for most surgeons, this system really reinforces the mission of P28 which is to provide the most comprehensive portfolio available. The innovation of P28 is forcing their competition to up their game. This product, while not a massive money maker, is what makes foot and ankle the most interesting market in al of orthopedics.
The system consists of 16 plates which are optimized to fit the bony anatomy of the midfoot and address conditions of TN and/or NC joint. The system provides 1.5- and 2.0 mm thick (Charcot) plates in talus to metatarsal and navicular to metatarsal spanning options. Surgeons may compress the plates using the Caspar which is available in the Gorilla® instrument tray or use the plates to span the fracture site. A novel Rasp is included to aid in preparation of the joint surfaces. The addition of the Gorilla® Central Column Plating System increases the number of plating options within the Gorilla® Plating System to 309 plates making it the most robust reconstruction and trauma foot and ankle offering on the market.

ACQUISITIONS
CONMED Acquires BIOREZ

One of our first featured, Little Guys, Big Ideas, has been acquired. Conmed, after purchasing In2Bones, has made another move and put up $85 million cash for Biorez, with an additional $165 million in additional payments based on sales growth. Biorez, based in New Haven, CT, has developed a proprietary synthetic ligament product called the BioBrace.

The BioBrace reinforces, regenerates, and then resorbs. Its biomechanical profile enables load-sharing to reinforce ligament and tendon repair throughout the healing process, while its biologic matrix acts as a scaffold to support tissue regeneration. The BioBrace provides supplemental strength for approximately 2 years before naturally resorbing.

While this will be a huge addition for ConMed’s sports team, specifically for ACL repair, it also adds another potential weapon in their new foot and ankle bag. Many of you are probably familiar with Artelon, if not - they have made a big push in marketing their FLEXBAND SOLO lateral ankle kit using a synthetic ligament product. 

ConMed, now with the added In2Bones team, will have the ability to create a very similar product using their existing Tenolok Anchor System. Since the BioBrace is already FDA cleared, it should be as simple as kitting the product up and hitting go on the marketing campaign. In addition to lateral ankle and deltoid repairs, the BioBrace could play a major role in Achilles tendon repairs as well. 

While this acquisition was made strategically for the knee and shoulder applications the BioBrace offers, it could pay major dividends on the foot and ankle side as well. Congrats to Kevin Rocco and the entire Biorez team.  

LITTLE GUYS, BIG IDEAS: SUTURE EDITION
SutureGard Medical


Closing acute wounds under tension is challenging. Suture can rip through the skin, second intent healing can take months with frequent dressing changes, and flaps and grafts have a high rate of complications, such as dehisence.

The HEMIGARD® ARS device was created to address these problems.

  • The HEMIGARD® bolsters skin strength to enable high tension linear wound closure without skin ripping.
  • The HEMIGARD® has 3 zones of action: (1) a rigid and waterproof zone with that faces the wound and will not rip under high tension, (2) a middle zone that is less stiff, but still waterproof, and (3) a trailing zone with low stiffness to lessen the risk of shear-induced blistering at the trailing edge.

HEMIGARD is the first of its' kind load transferring skin anchor that has reduced lower extremity postop wound breakdown rates by 80%.
Suturegard was founded by plastic surgeon, Dr. Dan Ladizinsky, MD. FAN stumbled upon Suturegard at AOFAS this year in Austin. Since then, we have followed their Instagram page and learned a lot more about the product. They have several case examples, mostly Foot and Ankle focused, featuring lateral extensile to anterior for TAR along with a lot of amputation applications.

WINTER INNOVATIONS

The innovation in EasyWhip™ is the patented two-part needle, which has a needle tip and a connectable rod/insert. The two needle components are fixed to ends of a length of ultra-high strength orthopedic suture. They can be connected to create a continuous loop of suture and disconnected to create a straight length of suture, which facilitates faster, easier stitching and creation of patterns that cannot be made with conventional needles.
Anyone who has done a FHL transfer or Lateral Ligament Repair with Allograft has used an Arthrex Fiber Loop. That product is easy to use and works very well. Winter Innovation, a sports med focused company, has added a slight variation and they think may have built a better mouse trap.

The idea that the Easy Whip can create a locking whip stich for some of these wimpy tendons that are often sent into the OR is a big differentiator. I’m not sure how much play this product will get in foot and ankle due to the apparent need of a graft station. It is unclear if the product can be used in sititue like the Fiber Loop for FHL or FDL transfer procedures.

They are picking a fight with a giant and may get squashed like a bug, but good for them for trying and innovating in this space. Congrats to Winter Innovations, they are one to watch.
