New Study Shows Value of MedWise Risk ScoreTM in PACE

In a peer-reviewed study of nearly 2,000 PACE participants (ptp), the odds of an Adverse Drug Event (ADE) were found to increase by 8.6% per unit of MedWise Risk Score (MRS). Further, each unit was valued at $1,037 in medical costs, including three emergency department visits and two hospitalizations per 100/ptp/year. The study, published in the journal Pharmacy, was a 12-month analysis of participants' medical claims data to determine participants' MRS, and correlations between the MRS and outcomes associated with ADEs.

The MRS is a prediction tool that helps identify participants at highest risk for ADEs, allowing for immediate medication management attention. Read an article by Ankur S. Patel, MD, MBA, FAAFP, Chief Medical Director, a former PACE Medical Director, on the MRS and how it can be used in practice.

PACE Case: Optimizing Pain Management and Deprescribing

An 89-year-old female PACE participant has a past medical history of rheumatoid arthritis (RA), urinary incontinence, peripheral edema, and gastroesophageal reflux disease (GERD). The participant also has limited mobility after falling and breaking her hip. Her daughter, who is also her primary caregiver, contacts the PACE prescriber to express concern over her mother's current medication regimen, especially her pain management. She states that her mother's pain mostly stems from RA and it is not relieved when given hydrocodone/acetaminophen. However, her mother does experience pain relief with the use of VoltarenĀ® gel, which is not regularly administered. The caregiver denies a diagnosis of depression or anxiety, and states that mood medications are used to control "sundowning" behaviors and agitation later in the day/evening. She adds that her mother's urinary incontinence is severe and still requires overactive bladder medication therapy. The participant's MedWise Risk Scoreā„¢ is 37 out of 50 (Very High).

CMS Guidelines for Reopening Healthcare Facilities

The Centers for Medicare and Medicaid Services (CMS) recently released a guide for reopening facilities to provide non-emergent, non-COVID-19 healthcare. The recommendations come as several states and regions begin to see a decline in COVID-19 cases and non-emergency healthcare begins to resume. The guide includes recommendations on safely providing in-person care, facility considerations, COVID-19 testing, PPE and supplies, workforce availability, and sanitation protocols.

Coming Soon: CareKinesis Chain of Custody App

CareVention HealthCare is launching a new chain of custody product, MedChain, for our Pharmacy Services (CareKinesis) partners. MedChain allows PACE centers to track medication deliveries from the time a shipment arrives at the center until the medication reaches the participant or caregiver. The app offers real-time functionality, so custody can be tracked at every point using a smartphone. MedChain will be available in late summer 2020. Coming Soon: Clinical Documentation Excellence Online Education

CareVention HealthCare is launching Clinical Documentation Excellence (CDE) Online Education. Available on-demand, 24/7, the course catalog focuses on the clinician's role in risk adjustment and the development of skills using ICD-10 Diagnosis Codes. CDE Online Education is designed for physicians, physician assistants, nurse practitioners, and other clinicians who offer services in value-based care settings, such as PACE. The curriculum, developed by the physician-consultants of Capstone Performance Systems, offers more than ten hours of engaging content, including timely, relevant topics such as risk adjustment methodology, the provider's role in risk adjustment, and documenting specific conditions such as heart failure and vascular disease. The series is offered as individual modules or subscription packages; a certification (CDE-C) option is also available. Tabula Rasa HealthCare University (TRHCU) will accredit course offerings for appropriate CE credits. In support of improving patient care, TRHCU is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC) to provide continuing education for the healthcare team. Welcome TRHC's New Chief Medical Director, Ankur Patel, MD, MBA, FAAFP

TRHC, parent company of CareKinesis, is pleased to welcome Ankur Patel, MD, MBA, FAAFP, as Chief Medical Director. Dr. Patel is a geriatrician and former Medical Director of Inspira LIFE in Vineland, New Jersey, where he practiced for four years. He also served as Medical Director for Population Health and Clinical Advisor for the Inspira Innovation Center in Mullica Hill, New Jersey. We are delighted to have engaged Dr. Patel to assist with our remote patient monitoring protocols and CareVention HealthCare data analytics, and to serve as an advisor to CareKinesis' Clinical Advisory Panel (CAP) of Medical Directors and TRHC University. He will work closely with Richard Schamp, MD, TRHC's Chief Medical Officer, to support clients during this transformational time in healthcare.
