Case Study: Further Reducing the Risk of ADEs in a Participant with a Low MedWise Risk ScoreTM
AP is an 85-year-old male with a past medical history of hypertension, benign prostatic hyperplasia (BPH) with an enlarged prostate, peripheral vascular disease, atrial fibrillation, and vitamin D deficiency. His MedWise Risk Score (MRS) at the time of the medication safety review was 6 out of 50, which is categorized as low risk. Despite the participant’s low risk, a polypharmacy consultation was requested by the prescriber to optimize the participant’s medication regimen and reduce the risk of adverse effects, such as a detrimental fall.
Scientific Study Shows Correlation of Elevated MedWise Risk Score with Premature Death
Researchers from the Tabula Rasa HealthCare (TRHC) Precision Pharmacotherapy Research & Development Institute (PPR&DI), in collaboration with DARTNet and the University of Colorado, recently published an original research article titled “Longitudinal Association of a Medication Risk Score With Mortality Among Ambulatory Patients Acquired Through Electronic Health Record Data,” in the peer-reviewed Journal of Patient Safety.

The study evaluated 427,103 health records and demonstrated that the MRS was able to identify medication regimens that put individuals at high risk for premature death, adjusting for multi-comorbidities and other conditions. The hazard ratio for those over 65 was 2.42, higher for the age group of 50-64 at 4.95, and even higher for younger age groups.

Having the MRS to help predict who is at risk for an adverse drug event empowers pharmacists and physicians, working together, to improve the safety of medication regimens. This also improves outcomes for participants, reduces utilization and total costs, and, as we now know, can prevent premature death.
