Portland Uncorked PACE Education and Appreciation Dinner

CareVention HealthCare invites you to Portland Uncorked! - a wine, beer, and cider tasting reception and PACE Appreciation Dinner on Sunday, October 21 in the Atrium Foyer and Ballroom at the Portland Hilton Downtown. 

Please join us as we introduce CareVention HealthCare, recognize Richard Schamp, MD, CEO Emeritus of Capstone Performance Systems, and present an award in his name.

The event will begin at 7:00 p.m. with a networking reception that will include the wine, beer, and cider tasting. Dinner will start at 7:30 p.m., and dessert and presentations will begin at 8:30 p.m.

Attendees will be able to obtain a "PACEport" that will allow participants to enter into a drawing to win a prize.

PACE Case: Glycemic Control

A PACE program nurse practitioner approaches a CareKinesis clinical pharmacist for help with a participant's anti-diabetic medication regimen. The participant is an 89-year-old female with a past medical history of chronic kidney disease stage 4, hyperlipidemia, type 2 diabetes, hypertension, GERD, and iron-deficiency anemia.
