PACE Case: Amputation Risk Averted

A 62-year-old female PACE participant has a past medical history of Type 2 diabetes, heart failure, urinary incontinence, hypercholesterolemia, obesity, and lower limb amputation. The participant was recently enrolled in a PACE facility and her prescriber requested that a polypharmacy medication safety review be conducted to re-evaluate her medication regimen and reduce her MedWise Risk Scoreā„¢, which was 19 out of 50 (moderate).

Understanding Part B vs. Part D Drugs for PACE Participants on Dialysis

The bundled Prospective Payment System (PPS) for renal dialysis services furnished to Medicare beneficiaries for the treatment of End-Stage Renal Disease (ESRD) went into effect on January 1, 2011. The bundled payment under the ESRD PPS includes all renal dialysis services furnished for outpatient maintenance dialysis, including drugs and biological products (with the exception of oral-only ESRD drugs until 2025), and other renal dialysis items and services that were formerly separately payable under the previous payment methodologies. New Medicare Number/Medicare Beneficiary Identifier

In April 2018, CMS beg
an mailing out new Medicare cards to all current Medicare beneficiaries, including PACE participants. The old Social Security Number (SSN)-based Health Insurance Claim Number (HICN) was replaced with the new MBI number on the updated cards. The switch to MBI numbers will help protect beneficiaries from identity theft and fraud. A s of April 30, 2019,  CMS has completed mailing new cards to all Medicare beneficiaries across the country. New Medicare enrollees will receive a card with an MBI number instead of a HICN.

During the transition period between April 1, 2018 and December 31, 2019, you will be able to use either the HICN or MBI on Medicare transactions. After the transition period ends, you MUST submit claims using MBIs, even for dates of service prior to January 1, 2020. For more information about the MBI, please refer to the  CMS FAQs.

Registration is now open for the CareVention HealthCare (CVHC) PACE Medical Director's Clinical Advisory Panel (CAP) on September 19 - 20, 2019 in Lake Nona, Orlando, Florida. The CAP, a physician user-group comprised of client Medical Directors and/or their designees, was formed in an ongoing effort to provide PACE professionals with optimal economic, clinical, and humanistic outcomes.

CareVention HealthCare Advisory Summit

CareVention Healthcare (CVHC) held its inaugural Advisory Summit for PACE Executives on June 20 - 21, 2019 in Philadelphia, Pennsylvania, and Moorestown, New Jersey. Initial survey results indicate that attendees found value in learning about our forthcoming advanced technology analytics, personalized medicine, remote patient monitoring, quality initiatives, and corporate culture. We were pleased to see the leadership networking opportunities unfold and listen to the valuable insight shared during the "It Works for Me" peer-education session.
