Here is the EVR template for the Spring Semester or Quarter 2 Term.

You will receive a separate email from Elena Salorio with complete instructions.
Please do not modify or change the template/tabs in any way, other than inputting your data on the second tab.

Award payments will be wired to the PC within 30 days after EVR submission.
