Melasma is characterized by brown to gray patches that commonly appear on the cheeks, forehead, and skin above the upper lip. It can cause significant distress and frustration, affecting psychosocial and emotional well-being because melasma is a chronic, complex, and relapsing condition.

There is no cure for melasma, but it can be minimized through dedication to treatment and protection using topical products, prescriptions, and a series of procedures.

We do not completely understand melasma in its entirety, but we know it most commonly affects women age 30 and older with darker skin types. Causes include genetics, hormones (oral birth control, hormone replacement therapy, pregnancy), UV light (sun), visible light (computer and phone screens, indoor lighting), medications, irritation, and heat.

Melasma involves sun-damaged skin, increased melanin production, blood vessel synthesis, inflammation, and oxidative stress. Due to these complex causes, treatment requires a multi-pronged approach.

Topical treatment options include hydroquinone, retinoids, tranexamic acid, azelaic acid, kojic acid, alpha arbutin, cysteamine hydrochloride, niacinamide, alpha hydroxy acids, Vitamin C, and ferulic acid. Prescription oral tranexamic acid may also be used in conjunction with other treatments. A series of low-energy fractional lasers and chemical peels can be effective for treating melasma.

Protection is essential and should include diligent use of a broad-spectrum SPF 30+ with a tint. Tinted sunscreens (like Sunforgettable® Total Protection™ Face Shield FLEX SPF 50 and EltaMD® UV Physical Broad-Spectrum SPF 41), often contain iron oxide, which helps protect the skin from visible light. Wearing a wide-brimmed hat in addition to daily sunscreen is also very important.
