IHS Seeking Comments on Proposed Reorganization of DDIGA Offices (including the Office of Tribal Self-Governance)

On October 10, the Indian Health Service (IHS) released a Dear Tribal Leader Letter announcing they are seeking comments and recommendations on the proposed reorganization of the IHS Headquarters Office of the Director, which specifically impacts the structure of offices under the Deputy Director for Intergovernmental Affairs (DDIGA). A key component of the proposed reorganization is the initiative of One IHS, which aims to improve effectiveness and efficiency across the Agency through greater coordination and collaboration. There are five primary objectives of the proposed reorganization, which adopts a One IHS model that: 1) strengthens IHS’ relationship with Indian Tribes and UIOs by streamlining operations and enhancing communications; 2) enhances the business relationships with Indian Tribes through self-determination and self-governance; 3) amplifies the work of the Agency with all partners by cultivating intergovernmental partnerships and furthering external relationships; 4) modernizes the way the Agency supports Tribes pursuing ISDEAA opportunities by supporting ISDEAA negotiation teams across the Agency working under a One ISDEAA model; and 5) fosters education and training throughout the entire IHS system on how ISDEAA interfaces with our everyday work. IHS will be hosting a virtual consultation session on November 14, 2024, from 2:00 – 4:00 PM ET. Congressional Corner

Congress is out for October recess and will return November 12th to begin to wrap up the 118th Congress in a lame duck legislation. The Congressional Diabetes Caucus leadership have announced new Dear Colleague Letters to demonstrate continued bipartisan support for the Special Diabetes Program and Special Diabetes Program for Indians. Reps. Diana DeGette and Gus Bilirakis on the House side and Sens. Susan Collins and Jeanne Shaheen plan to release their respective letters this week. NIHB will be coordinating to obtain signatures and encourage Tribal advocates to do the same.


While legislators are home in district engaging for constituents before the election, use the opportunity to raise important legislative priorities for Indian Country! NIHB has compiled advocacy resources to help:


  • Mandatory Funding for CSC and 105(l) Leases: NIHB will continue advancing our advocacy efforts regarding moving Tribal Contract Support Costs (CSC) and 105(l) Lease Payments to mandatory appropriations this month. This includes getting Members of Congress to sign onto letters to leadership supporting this change. Visit our advocacy resource page for more information and to see how you can get involved! 
  • SDPI Renewal: The Special Diabetes Program for Indians (SDPI) expires on December 31, 2024. Click here for additional resources supporting renewal of this critical program.

IHS October 10 Letter: The Director writes to seek comments and recommendations on the proposed reorganization of the IHS Headquarters Office of the Director.


IHS October 9 Letter: The IHS Director writes to Tribal Leaders to invite them to provide updated Facility Master Plans and other identified health care facility needs to be considered for inclusion in the 2026 IHS and Tribal Health Care Facilities’ Needs Assessment Report to Congress.


IHS October 8 Letter: The IHS Director writes to Tribal Leaders to announce the new solicitation for the FY25 IHS Joint Venture Construction Program (JVCP).

Treasury September 13 Letter: Consultations on a Notice of Proposed Rulemaking (“NPRM” or “Proposed Rule”) entitled “Tribal General Benefits” (REG-106851- 21).


SSA September 13 Letter: The Director writes to share an opportunity to attend the Social Security Administration (SSA) Virtual Tribal Consultation on October 15 or October 17.

SAMHSA September 12 Letter: The Substance Abuse and Mental Health Services Administration (SAMHSA) announces it has been working to modify its Client-level Government Performance Results Act (GPRA) instrument for the Center for Substance Abuse Treatment (CSAT) and the Client-level National Outcomes Measures (NOMs) instrument for the Center for Mental Health Services.
