BETTER HEALTH PATHWAY HUB


 For the past 3 years Carmella Rose Health Foundation has been a strong proponent of establishing a Certified HUB Pathway model for Cuyahoga County. Carmella Rose has been involved in a pilot HUB program out of Mansfield, Ohio for almost 2 years now….with great results. We have gained a lot of experience with this model and are now happy to share our expertise and experience with Better Health Partnership to build the Cleveland model. BHP has taken the lead and will spearhead this effort to establish a Pathways Community HUB in Cuyahoga County by February of 2020. The model features coordination of care by home visiting services provided by a certified Community Health Worker (CHW’s) for at-risk populations with complex health and social needs. This model has a reimbursement piece from the Managed Care Organizations for completed pathways which allows some sustainability for the agencies that contract with the HUB.

Meet our Summer Intern
Shannon Kokish is an intern at the Carmella Rose Health Foundation in Ohio City, Ohio. She received her bachelor’s degree from Ohio University in Health Care Administration while minoring in business this past May. During her time at Ohio University she was a club member of OUSHA, Ohio University Society for Health Administration. Aside from interning at Carmella Rose, she serves and bartends at a family restaurant on the east side and nannies as well. Once she finishes up her internship, she plans to move out west to Denver, Colorado, perusing a career in the medical field. In her down time, she enjoys hiking, singing, and working out along with many other hobbies.
