🌟 A Special Day for Loretta 🌟

Teamwork and Person-Centered Planning in Action

It was a special day for Loretta as she gathered with her Spectrum Support Coordinator, residential provider, day program provider, and BGS guardian for her annual Individualized Service Plan (ISP) meeting. The meeting, thoughtfully coordinated by Spectrum, brought together everyone on Loretta’s team to celebrate her progress and plan for the year ahead — with her voice leading every step of the way.

Spectrum’s Support Coordinator, Imer, took the lead in preparing for the meeting by gathering input from all team members, reviewing Loretta’s goals and outcomes, and ensuring that her preferences and choices shaped the agenda. During the meeting, the SC guided the discussion in a way that empowered Loretta to express what matters most to her — from the activities she enjoys to the supports that help her succeed each day.

Throughout the process, the SC modeled Spectrum’s core values of collaboration, person-centered planning, and respect, ensuring that all providers worked together to strengthen Loretta’s plan of care. By creating an open and supportive atmosphere, the SC helped transform what could have been a routine meeting into a moment of genuine celebration and teamwork.

At the end of the meeting, Loretta proudly shared how good it felt to hear such positive feedback from her team. When the day program provider suggested taking a group photo, Loretta happily agreed — capturing a moment filled with smiles, connection, and pride.

At the Support Coordinator’s next visit, Loretta eagerly greeted her and proudly displayed the framed photo from her meeting — a daily reminder of her accomplishments and the teamwork behind her success.

Moments like these reflect Spectrum’s mission in action: empowering individuals through meaningful relationships, compassionate support, and collaborative planning that celebrates every person’s journey.
