Case Log Information



Please continue to log all your cases through the OTA database. You will have access to your fellowship case logs through August 7, 2020. After that time, you will need to contact your fellowship program coordinator or [email protected] to receive your case log reports. Contact [email protected] with any questions or to add CPT codes that are not currently found in the OTA database.
