New education poster helps patients keep health info updated
Coordinating care and communicating with patients are critical components of improving the health of populations. In the patient centered medical home care model, primary care physicians serve as central coordinators of care for patients.

The Physician Alliance created a new poster for primary care practices to help engage patients in their care. This patient check list poster reminds patients to talk to the practice staff about a variety of information, including:
  • alert the practice of any tests, new medical diagnoses or other updates to their health since the last visit
  • ask the office about after hours care or recommended urgent care facilities
  • schedule follow up visits, immunizations/vaccines or other screenings prior to leaving
  • and more

Practices are encouraged to place the poster in waiting areas and exam rooms. TPA practices can download and print, or order printed copies, from the website .
