RE-ENROLLMENT & NEW REGISTRATION

Dear MAP Families,

It's time to complete the 2025-2026 MAP RE-ENROLLMENT & NEW REGISTRATION forms. Please read the important information below and click on the appropriate button for the form you need. Forms are DUE BACK TO MAP on or before Friday, March 7, 2025.

RE-ENROLLMENT

NOT RE-ENROLLING for 2025-2026?  Please email Annette if your child will not be returning to MAP. Refundable Tuition Deposits will be returned at the end of the school year, provided you have followed our tuition policy and your account is paid in full.

Children's current MAP schedules will be honored going into the 2025-2026 school year. Additional days or changing days are subject to space availability. MAP will confirm schedules for 2025-2026 in April. Dropping a day(s) after 2025-2026 enrollment confirmations, will result in loss of the day(s) for 2025-2026. 

Annual registration fees will be billed with your March 14th tuition statement ($50 registration fee, $30 for each sibling).  MAP's tuition policy is available on our website: www.medfieldafterschoolprogram.com. 

Refundable Two Week Tuition Deposits: No additional refundable two week tuition deposits will be due for families confirmed for their same schedule. If a new sibling is joining MAP, the refundable two week tuition deposit will be billed on a future tuition statement. Adding or Dropping Days? Adjustments will be reflected on a future tuition statement. Families withdrawing from MAP will have their two week tuition deposit returned at the end of the school year, provided they have followed our tuition policy and their account is paid in full. 

NEW to MAP? NEW REGISTRATION FORMS Below

Important second step: New families must mail a $50.00 registration fee check ($30 for a sibling) to The Medfield Afterschool Program, P.O. Box 18, Medfield, MA 02052. Registrations will not be considered complete until MAP has both the registration form and registration fee.

CURRENT MAP FAMILIES registering a new sibling will be billed their registration fee with their 3/14 Tuition Statement.

($50.00, for the first child and $30.00 for each sibling)

MAP does its very best to accommodate families' requested schedules. Current families have enrollment priority over new families. If new registrations exceed the space available, MAP will conduct a lottery. If you have any flexibility with your requested schedule, please be sure indicate that on the registration form.

All families will be notified via email in April of one of the following: 

• confirmation of their requested schedule
• confirmation of some of their requested schedule & wait listed for some days
• wait listed for their requested schedule

New Families are required to send MAP a refundable two-week tuition deposit based on their MAP schedule. Failure to send the tuition deposit by the deadline will result in being removed from our enrollment.

Friday, March 7, 2025

For NEW Families: Both the registration form and fee must be received/postmarked by the above date. Incomplete or late registration are subject availability.
