  1. Ask every individual about their tobacco use at every visit OR see how the quit attempt is going.
  2. Advise individuals who use tobacco that quitting is one of the most important things they can do for their health.
  3. Assess willingness to attempt to quit.
  4. Assist in quit attempts by offering support at every encounter and refer individuals to evidence-based cessation treatment, including counseling and medication.
  5. Arrange follow-up with practice and referrals to additional cessation resources, including quitlines.
