Open Enrollment will be Completed on TeamOne Logistics Employee Portal All employees must log in to the TeamOne Logistics Employee Portal to enroll, change, confirm, or waive benefits by February 15th. This is the same Employee Portal where you access your pay stubs. Make sure you have your Username and Password ready!

New Insurance Provider for Medical, Dental, Vision, HSA, EAP, and your voluntary benefits. Anthem Blue Cross Blue Shield will be the administrator for your medical, dental, vision, HSA & EAP benefits You will receive new ID cards in the mail Sydney mobile app or Anthem.com will be your one-stop to managing your benefits Anthem Blue Cross Blue Shield is our New HSA Administrator. The Hartford is our new carrier for supplemental benefits for Accident, Critical Illness, and enhanced Short Term Disability. TRANSITION TO ANTHEM BLUE CROSS BLUE SHIELD If you are currently covered on one of the TeamOne Logistics medical benefit plans, please note the impact of our transition from HealthComp/Cigna to Anthem (effective 3/1/23) on current care: New ID Cards If you enroll in the Anthem Blue Cross Blue Shield plans, new ID cards will be mailed to your home. You will need the Member ID# on the card to create an account at Anthem.com and access your benefit information.  If you have not received your ID card by March 1st and need to log-in for prescriptions or services, contact The Benefit Butler at 404-991-6070 to get your Member ID number. Contact the Benefit Butler about any ongoing therapy or scheduled procedures.   Medications – If you are on a maintenance medication or a prescription that requires authorization (prior authorization, step-therapy, etc), you are encouraged to have sufficient medication for treatment through the first week of March. Ongoing therapy or scheduled procedures – If you are in the middle of a therapy regimen or have a procedure scheduled for March/April, please complete Anthem’s Transition of Care form found on the TeamOne Logistics Employee Portal.  Contact the Benefit Butler about any ongoing therapy or scheduled procedures.

Required Annual Notices The Required Annual Notices for our company's benefit plans are posted on the TeamOne Logistics Employee Portal. These documents, which we are legally required to share every year, include information about your benefits and your rights surrounding them. The Prescription Drug Coverage and Medicare Part D Notice is included. You may need this if you are enrolling in Medicare. Paper copies are available upon request at no charge.
