Member Survey Questionnaire

The Texas Oral Health Coalition (TxOHC) Board of Directors (BOD) would like to gain input from the general membership as to whether individual and/or organizational member annual dues should be charged.

A survey questionnaire was included in summit packets and the issue was raised and discussed during the annual meeting.  Attendees were able to submit their completed questionnaires during the summit, but the BOD wanted to give members not able to attend the summit an opportunity to weigh in.

Please complete the survey and return to Beth Stewart before December 31, 2013.
