Dear Colleagues,
Summertime is when we welcome our new UCSF residents, students and fellows to ZSFG. We are very proud of our trainees and our training programs from the Schools of Medicine, Pharmacy, Nursing, Dentistry, and Graduate Studies at ZSFG. Many of our faculty, myself included, completed UCSF training rotations at ZSFG.
In reflecting on current training during the COVID-19 pandemic and my own memories as a resident during the beginning of the HIV/AIDS epidemic, I see many parallels and differences.
Initially, we had no idea what was wrong with the young men who presented with multiple opportunistic infections and a previously rare tumor known as Kaposi sarcoma. As the epidemic progressed, the underlying cause was defined and providers learned together how to identify and care for these patients. This learning resulted in the first AIDS ward in the country as shown in the

documentary (below) and the establishment of the San Francisco Model of Care still practiced in
Ward 86
. Exposure of ourselves and our families was a big concern. We began using gloves for our own safety and the safety of the patients. Ironically, this change was as politically sensitive as masks are today, some thought wearing gloves was disrespectful to our patients. Now gloves are considered universal precautions. As we quickly learn more about COVID-19, we know asymptomatic transmission of the virus is distinctly possible and we must use masks, social distancing and hand washing when interacting with everyone.
Many who trained during the HIV/AIDS epidemic made careers in that field, caring for, researching and developing new treatments and best practices. This month,
Monica Gandhi
, who trained at and continues to work at ZSFG, co-hosted
AIDS2020
, a landmark conference. I expect some of our current trainees will find a similar purpose working with COVID-19.
Another major challenge for our trainees and training programs is the cultural and systematic change of assuring reasonable duty hour standards. The terms Housestaff and Intern are carryovers from the days trainees expected to live in the hospital and work around the clock. Necessary adjustments to these exhausting conditions has affected and will greatly change our training programs going forward.
Conditions during these exceptional times will undoubtedly shape our training programs and prepare our trainees for their roles as doctors and leaders of the national conversation around health issues, policies and research. Please join me in welcoming the incoming trainees and appreciating and celebrating those outstanding physicians who continue their training here.
