Changes in MassHealth: Work Requirements & Six-Month Eligibility Checks

  • With big changes due to OB3, some MassHealth constituents may be required to verify every 6 months.
  • New requirements may require some members to renew more frequently.
  • New changes may require MassHealth to check eligibility for certain members every six months and to require “community engagement,” which includes work, education, or volunteering, for some members.
  • This may begin as early as the Fall of 2026.
  • These changes will primarily affect adults ages 19–64 who do not have young children and do not have a disability. Several groups are exempt, including:
  • Parents or caregivers of children 13 or younger or of a person with a disability
  • Pregnant and postpartum individuals
  • People who are incarcerated or recently released (within 90 days)
  • Current and former foster youth under age 26
  • Individuals who are medically frail

MassHealth estimates that about 175,000 members may lose coverage because of these new requirements. Loss of coverage will occur gradually, and the state is still awaiting federal guidance from CMS to understand the full scope of the policy.

Lyme Disease Update: New England

The Northeast continues to experience the highest rates of tick exposure in the United States, with increased emergency visits for tick bites across the region. Recent surveillance in New England shows rising tick populations, with some areas reporting very high densities and >50% infection rates in adult deer ticks. Experts attribute this trend to warmer winters and environmental changes, describing the current season as a “hot tick year.” Massachusetts, New Hampshire, and Maine remain among the most affected states.

Signs & Symptoms of Lyme Disease

Early (3–30 days after tick bite):

  • Expanding erythema migrans (“bull’s-eye” rash; often uniform red)
  • Fever and chills
  • Fatigue
  • Headache
  • Muscle and joint aches
  • Swollen lymph nodes

Early disseminated disease:

  • Multiple rashes
  • Facial palsy (CN VII)
  • Severe headache or neck stiffness
  • Migratory joint pain
  • Palpitations (possible cardiac involvement)

Late disease:

  • Recurrent or persistent arthritis (often knee)
  • Neurologic symptoms (neuropathy, cognitive changes)

Treatment:

Common first-line antibiotics include:

  • Doxycycline
  • Amoxicillin
  • Cefuroxime axetil

Early recognition and prompt antibiotic treatment are key to preventing complications.

As of February 23, 2026, the FDA mandated a Risk Evaluation and Mitigation Strategy (REMS) for Nexplanon to ensure proper insertion/removal and reduce complication. All healthcare providers must complete new, specific certification training by August 23rd, 2026 to continue inserting/removing the implant (which is now approved for up to 5 years of use). If you have inserted/removed Nexplanon devices in the prior 3 years, and plan to continue on doing so, you are required to register with REMS and complete the registration/knowledge assessment/REMS enrollment by using the link below.

https://www.nexplanonrems.com/healthcareproviders/register

If you have not inserted/removed in the prior 3 years (and plan on doing so in the future), the FDA requires you complete an in-person training to be able to insert/remove them; please contact Kristina Gracey (Kristina.Gracey@umassmemorial.org) as we work to organize in-person trainings for all relevant providers needing this.

Please nominate a student for the Ann Nemitz Award

Do you know of a graduating medical student going into Family Medicine who stands out as deserving of the Annual Ann Nemitz Award? The spirit of this award emanates from the way Ann Nemitz practiced medicine. Her guiding principle, from which all of her behavior followed, was that it was truly "a privilege to serve" others. Please respond to Mike Ennis

Michael.Ennis@umassmemorial.org. A list of this year's students who matched in family medicine available here.

Topics will include addressing bias in the clinical and learning environments, ambulatory procedures, intrapartum care, and integrating learners into scholarship. Now is a great time to ensure that your clinic schedule is clear! We have been awarded a grant to evaluate AI use in clinical practice in the primary care setting. This project entails the participant to complete an 8-9 question multiple choice pre survey. A subset will be asked to primarily use a specific AI tool for their clinical needs but may still use other tools like websites, other apps or books afterwards. After 2 weeks, all participants will again complete the survey. For their participation, all will receive a high end insulated travel mug (like a Camelback or Yeti). Please share this invite with all primary care providers in your system, including Family Physicians, Primary Care Internists, and Nurse Practitioners and Physician Assistants who practice primary care. If interested, please sign up by registering your name and email at link below:
