Dear Colleague,
The email below was distributed at 8 am this morning, but it did not allow access to the survey. The problem has been fixed and you should be able to access the survey from the link below. We apologize for the inconvenience.
As an advocate for the health care needs of individuals with intellectual and developmental disabilities (IDD), we are often contacted by those seeking a medical, dental or mental health/behavioral health provider. Not all practitioners have the expertise or experience to properly treat a person with IDD and so we have, over time, compiled a list of health care providers who have been recommended to us.
It has been a number of years since we've updated our list and so we are asking for your assistance as we work to keep our database current. With that in mind, if you would like to recommend a medical, dental, or mental health/behavioral health provider, please complete the brief survey below. Your participation will help people with IDD seeking health care. Additionally, if you or someone you know is in need of a practitioner, whether it be for medical, dental or mental health/behavioral health issues, please reach out to Jessica Sergio at Jsergio@arcnj.org and the Mainstreaming Medical Care Program will try to direct you to health care providers for your loved one with IDD.
Please note that the names in our database have been recommended by other family members. The Arc of New Jersey will share names of some health care providers with the understanding that we are not able to independently verify or confirm the level of experience or expertise of the providers who have been recommended to us.
