Self-Assessment Questionnaire Screening:
Green or Red QR Codes - What Do I Do Next?
Screening is an important tool to keep our campus safe. Students, faculty, staff and visitors must complete the daily self-assessment questionnaire prior to attending campus.

The Safe Lancer App makes completing the questionnaire a quick and easy process. No mobile device? A paper form is also available.

Upon entry of a building or designated campus space, you'll quickly and easily scan your green (approved) Safe Lancer App QR Code (badge) at QR Code scanners positioned at designated building entrances. Building maps demonstrating the location of these entrances are available on the Return to Campus website.
RTC Student Ambassadors will be present on campus to assist and/or may request to see your badge. You must scan every time you enter a campus building or space. This assists with contact tracing. Please help avoid bottlenecks by having your badge ready to scan prior to entering the building.

Remember, vaccination is mandatory to attend campus. All UWindsor students, faculty, staff and visitors must complete a COVID-19 vaccination attestation. The daily self-assessment questionnaire requires a confirmation code that you'll receive by completing the attestation or upon receiving a negative test result from on-campus COVID-19 testing.
What do I do if I receive a Red (Denied) QR Code?
A red (denied) badge results from your responses to the COVID-19 screening questions and/or an incomplete COVID-19 vaccination attestation. To attest your vaccination status and upload proof of vaccination (if applicable), visit uwindsor.ca/vaccine. You will need your vaccination certificates and your UWinID and password.
Take the following steps if you receive a red (denied) badge:

  1. DO NOT proceed to campus. Stay home and self-isolate.
  2. Complete the Ontario COVID-19 self-assessment. Follow the directions provided.
  3. Follow these instructions to report an illness and receive accommodation through UWinSite.
