The American Academy of Home Care Medicine (AAHCM), the Duke-Margolis Health Policy Center, the Home Centered Care Institute, and the National Home-Based Primary Care Learning Network seek input from home-based primary care (HBPC) practice leaders and clinicians on issues associated with the possibility of developing a practice-level certification process for HBPC practices to elevate the field of HBPC.
This would be akin to the patient-centered medical home practice certification process used in office-based internal medicine practices. We are conducting virtual (via Zoom) focus groups of key stakeholders in HBPC to obtain input on this issue.
We are particularly interested in the views of HBPC clinicians. The Home Centered Care Institute is offering $50 per 30 minutes of participation for this one-time focus group.
