Colorado Opioid Model to Advance Treatment (CO-MAT) Project Resources

The Denver Health CAM Academy (opens webpage) has released a free Opioid Use Disorder (OUD) Hub and Spoke Toolkit (opens webpage), designed to strengthen Colorado’s systems of care for individuals with OUD.

This toolkit builds upon the report shared last year (opens PDF) that assessed the current state of referrals and access to OUD treatment services.

The Hub and Spoke Toolkit provides:

  • Implementation Framework: Step-by-step guidance for developing, launching, or strengthening a hub and spoke system.
  • Adaptable Resources: Templates, workflows, protocols, and tools that organizations can easily tailor to local needs.
  • Case Studies: Real-world examples from Colorado and beyond demonstrating how Hub and Spoke systems improve access, coordination, and patient outcomes.
