Objectives of the day
1. To augment current knowledge of ophthalmic conditions commonly seen by family
physicians
2. To gain a better understanding of the management/treatment of these eye conditions
3. To foster a collaborative approach (between family medicine and ophthalmology) to
treating ophthalmic conditions.
4. To learn about important clinical skills that can be used in the family physicians office.

How to Participate in/Lead Coordinated Care Planning Training
This interactive training will walk participants through the Coordinated Care Plan process including how to identify, engage and interview individuals who would benefit from Coordinated Care Planning. While maintaining the emphasis on interactive learning and networking opportunities, we have managed to condense the training into a half-day session!
