We are pleased to announce our upcoming CME event, Frontline Excellence in Ambulatory Chronic Disease Management, a complimentary three-module series designed to bring you up to speed on multidisciplinary approaches to chronic condition management, with attention to comorbidities and complex cases in the outpatient setting. The first module, focused on congestive heart failure and diabetes mellitus, will cover medication considerations, multidisciplinary team care, and HF subtypes. We encourage PCPs, cardiologists, endocrinologists, mid-level providers, CDEs, and RNs to attend. Keep an eye on your inbox for a formal invitation and registration information in the upcoming weeks.

Check your email inbox tomorrow, September 24th, for your annual Provider Experience Survey. We value your feedback as it helps drive many of our strategies to improve services, outreach, and communications. For the first time, we will also be surveying your office manager to gain a better understanding of their communication needs. They will receive their survey tomorrow as well.

MSHP Uses CAQH Proview to Access Practitioners’ Data for Credentialing—Keep Your Profile Up to Date
We require that all members of the IPA have an active profile and authorize MSHP to have access their proview provider profile. MSHP will access practitioners’ profile during the initial credentialing process or initial membership enrollment, as well as every 36 months for recredentialing. We highly recommend that all members keep their attestation up to date. CAQH requires providers in New York State reattest every 120 days. This will reduce inquiries from the MSHP staff in reference to reattestation of your profile.

Sign up to Receive Inpatient/ED Alerts for your Patients
Be in the know every time your patients are admitted or discharged from the hospital or the ED by subscribing to the Subscription Notification Management web tool on MSHP’s Provider Portal. The tool is designed to allow you to self-select your preferences to receive real-time patient hospitalization alerts sent directly to you via EMR direct message, email and or/text message.

Commonly Missed Diagnosis: Peripheral Vascular Disease
PVD remains one of the top overlooked diagnoses during routine physical exams. CMS recognizes PVD as a high-risk medical condition. Common physical indicators are signs such as: decreased pedal pulses, skin that is cold to touch, loss of normal skin texture, hair loss on legs and feet, leg muscle pain and cramping, numbness, and poor wound healing. Risk factors also increase in patients above the age of 60, and in patients who smoke, and in those who have atherosclerosis, hypertension, high cholesterol, or diabetes. Be sure to assess patient for any signs and symptoms, and if they do have PVD, document the diagnosis in the Plan of Care.

Updates and Revisions to the CPT Code Set
The AMA has made several recent updates to the CPT Code Set including:
- E/M office visit modifications intended to simplify coding and documentation, proposed for adoption by CMS on January 1, 2021
- 329 editorial changes, 206 new codes, 54 deletions, and 69 revisions

COVID-19 additions and revisions that have been approved for immediate use and published for the 2021 CPT code set
