Learning and trying out life skills is an important step for students as they plan to move into adulthood. For many students with special health care needs and/or disabilities, planning for that transition can include developing skills needed to manage their own healthcare.

*Webinar 2: Designing Value-Based Payment Innovations for Pediatric-to-Adult Transitional Care: Lessons from the Field*

The National Alliance to Advance Adolescent Health will host a webinar to: 1) discuss a step-by-step approach to designing value-based payment (VBP) innovations for pediatric-to-adult transitional care, and 2) hear directly from senior officials in two large Medicaid MCOs – Florida Sunshine Health and Texas AmeriGroup – about their own experiences with designing a transition VBP pilot. This webinar is intended to assist state Medicaid agencies, commercial insurers, health plans, and pediatric and adult practices to expand their value-based efforts around pediatric-to-adult health care transition. About Got Transition
Got Transition is a program of The National Alliance to Advance Adolescent Health and is funded through a cooperative agreement from the federal Maternal and Child Health Bureau, Health Resources and Services Administration. Its aim is to improve the transition from pediatric to adult health care through the use of evidence-driven strategies for clinicians and other health care professionals; public health programs; payers and plans; youth and young adults; and parents and caregivers.
