New CPT Codes for Immunization Counseling

Starting January 1, 2026, new CPT codes will be available to capture and bill for immunization counseling when a vaccine is not administered on the same date of service. 

New Time-Based Counseling Codes



• 90482 – Immunization counseling by a physician or other qualified health care professional when immunization(s) is not administered by the provider on the same date of service; 3 minutes up to 10 minutes, wRVU = .24
• 90483 – greater than 10 minutes up to 20 minutes, wRVU = .50
• 90484 – greater than 20 minutes, wRVU = .75

Please note that time-based codes are reported only when documentation supports the time spent, distinct from time providing other services. A 25 modifier should be appended to the E/M visit code to indicate that the visit is a separately identifiable service from the immunization counseling. 

Practice leaders should verify payment rates have been established by major payers for these new codes as a part of providing these services and billing the new codes.
