Hello Explorer and PreK Families,

We hope you are enjoying your summer. The Ohio Department of Children and Youth no longer requires annual Health History Forms to be completed by your children’s physicians. You will complete all assigned forms in our Final Forms health portal and upload a copy of your children’s most recent vaccination records (watch short instruction video). As a reminder, your children’s physicians will need to complete the necessary additional documents if your children have the following conditions (documents hyperlinked): asthma, requires an inhaler at school, diabetes, allergies and requires an epi pen or requires prescription medication while at school. We appreciate your partnership.
