An optional update to the ISP form has been released in July. Translated and updated Large Print forms have also been released. This email contains details about these updates.

7/2019 Release of Updated ISP form with Translations

Oregon's Office of Developmental Disability Services (ODDS) has released an update to the ISP form (SDS 4118). This version includes the latest available service codes, an update to the risk management plan, and other minor formatting fixes. Here is a one-page summary of the changes.

The 7/2019 update is considered optional, meaning ISP renewals do not have to be transferred over to the new version. See the list of valid versions of the ISP form for more information.

The 7/2019 ISP form has also been translated into several languages. New translations have been released in Arabic, Korean, Russian, Somali, Spanish, and Vietnamese. Special thanks to our volunteer reviewers in the field for their help with these translations!

The Large Print English version of the ISP form has also been updated with this release.

For case management staff who want to transfer data from an earlier version of the ISP to the 7/2019 version, improved export/import features are available to those with a licensed version of Adobe Acrobat DC. A new import/export tip sheet is available.
