Population Health Trust is asking all Skagitonians to take Washington state's Community Recovery-Oriented Needs Assessment (CORONA) survey to help researchers and policy makers understand the behavioral, economic, social and emotional impacts of the COVID-19 pandemic. Your responses will help the Washington Department of Health and other state agencies include many perspectives as they plan for recovery. Please take the survey and encourage your clients to fill it out as well. It took me less than 5 minutes to fill out.

  • You do NOT have to provide your name or personal information that can identify you. (They ask these questions but they can be skipped.)
  • Survey is in 6 languages. Additional languages including Mixtec and Triqui are available by calling the phone number and taking the survey by phone with interpretation.
  • If you choose to provide contact information (text, email or phone), you will be entered into a raffle for a $100 Amazon gift card.
