Dear Self-Directing Participants,

On July 1, 2024, DDA updated their reasonable and customary rates for Self-Directed Services. Charts are now available for services provided by staff, vendors, and providers. Please note that some services can only be provided by an entity that is approved as a DDA certified provider as noted below. When requesting reimbursement for one of these services, please submit a copy of the DDA approval letter for the provider to fmsvendor@thearcccr.org. 

For Day Habilitation services only, please also submit proof of OHCQ licensure. These documents may be submitted with an initial reimbursement request or prior to the provision of the service. Documents only need to be submitted once per vendor, provided they are up to date. 

Commonly used services that require DDA certification include:

• Behavioral Support Services
• Day Habilitation
• Employment Services-Discovery
• Employment Services-Job Development

Less frequently used services that require DDA certification include:

• Family and Peer Mentoring
• Family and Caregiver Training and Empowerment
• Housing Support Services
• Participant, Education Training and Advocacy
• Remote Support Services
• Respite Care-licensed site
• Supported Living
• Transition Services
