Urban Family Practice to hold huge health fair on Niagara Street
Urban Family Practice (UFP) is bringing back an old favorite – this time supersized amidst Niagara Street’s current revitalization. On Saturday, July 21, 2018, from 12:00 p.m. – 6:00 p.m., UFP will close Niagara Street between Jersey Street and Pennsylvania Avenue for the 2018 Urban Family Practice Health Fair. The fair will celebrate the community and the important role Urban Family Practice has played on the West Side of Buffalo for over 20 years. Buffalo Mayor Byron Brown will be in attendance and will speak around 2:00 p.m.

The event will feature approximately 25 health care vendors and several health screenings including blood pressure, blood glucose testing and BMI. There will be food, music, and chances to win bikes, iPads and huge plastic bins filled with needed household items such as towels, pots and pans, cleaning products, dishes and more. A family-friendly event, there will also be plenty of children’s activities, including: pony rides, face painting, a bounce house and a cotton candy machine.

Dr. Raul Vazquez, CEO of Urban Family Practice (UFP) and G-Health Enterprises, donated
bicycles to every child in attendance at the Urban Family Practice at Jefferson open house on
Saturday, June 2, 2018. Dr. Vazquez gave away twenty-one bikes, much to the excitement
of the kids. 

The celebratory open house also featured television giveaways, free bike helmet giveaways
(courtesy of the Buffalo Police Athletic League), free lunch vouchers, healthy cooking

In June 2017, some 7,000 patients received a letter stating that their neighborhood primary care
practice at Jefferson Avenue and East Utica Street was closing its doors, leaving these individuals
without a convenient, close-to-home, family doctor. Dr. Raul Vazquez stepped in and purchased the clinic, expanding his busy West Side primary practice to the East Side. 
The NAACP recently honored Dr. Raul Vazquez for his outstanding service to people in the community.
Dr. Raul Vazquez talks to students from D’Youville College’s Health Services Administration. Dr. Vazquez was the keynote speaker at the college’s Upward Bound Science, Technology, Engineering and Math (STEM) conference on May 17, 2018.

Dear Dr. Vazquez,

I want to say thank you for everything that you are doing for those that are in need of health answers and results. I am one of them that was in need of guidance in nutrition and exercise.

I have been asking my doctor for years if he could start up a program that would help diabetics to eat the proper food and a facility that would allow those who cannot afford to pay for a gym membership a place to workout.

So when I was approached by one of the navigators while at a doctor's appointment, I first was guarded. I was already dealing with many doctors and did not want to add anymore to my already full schedule.

Since I have been in the GBUAHN wellness program, I have changed my eating habits and have lost weight that has been so hard to get off by myself.

The staff has been a big help in working with me and keeping me focused. At one point in my life I thought that I was not going to make it, but when the team spoke encouraging words to me, it gave me hope. I thank Sam (my navigator), Belinda, Andre and Dana - they are all my support system; as well as those who have left (I thank them too) and you, Dr. Raul Vazquez, for everything who you have done for me those and who did not have a way to get the real help that is needed in our lives, YOU ARE A REAL LIFE SAVER.

The Renewal Process for Erie & Chautauqua County YourCare Option (Medicaid Managed Care) Members is Changing!
If you treat and/or know YourCare members living in Erie or Chautauqua Counties, please help us communicate that their renewal process is changing! Previously, these individuals may have recertified with their local Department of Social Services (DSS), but now will be required to renew through the NYSOH Health Plan Marketplace.
YourCare Marketplace Facilitated Enrollers can help your patients through this process!
Members may renew right over the phone! The dedicated YourCare enrollment line is 1.866.273.6121. If your patients would like to meet with us, please let them know to stop in or make an appointment at one of our two enrollment centers – 446 Main St., Buffalo, 14202 or 6400 Sheridan Dr., Williamsville, 14221. We are open Monday – Friday, 8AM – 5PM.
In Chautauqua County, your patients may call Penny Gullo, Marketplace Facilitated Enroller, for help with recertification at 716.200.3058.
