Attend any healthcare conference and you’ll quickly discover that it’s become downright fashionable for healthcare leaders to talk about their unwavering commitment to “value-based care.” The expression has become ubiquitous in healthcare circles. Its virtuousness goes unchallenged. But should that be the case? Michael E. Porter and Elizabeth Teisberg (with whom I worked as a student and researcher at Harvard Business School from 2006-2010) popularized the value equation (Value = Quality/Cost) and suggested that improving value should be any healthcare system leader’s highest aim.
The Pharmacy Quality Alliance (PQA) has a new pharmacy performance measure that evaluates adherence to medications for diabetes, renin angiotensin system antagonists for hypertension, and statins for high cholesterol through a single composite measure. Although many might think the PQA already had member-endorsed adherence measures for these medications, the situation is a bit nuanced. There are 3 similar PQA adherence measures used in the Medicare Part D Star Ratings that were developed, tested, and specified for health plan performance measurement.

Even before the COVID-19 pandemic, nurses around the country reported feeling overworked and burned out. Burnout, characterized by energy depletion or exhaustion, job-related feelings of negativism or cynicism, and reduced professional efficacy, is not unique to nurses, with other types of health professionals and industries also experiencing high rates. The phenomenon is so common that the World Health Organization (WHO) included burnout in the International Classification of Diseases (ICD-11) as an occupational phenomenon. In a recent survey, the American Nurses Association (ANA) found that 62% of nurses report symptoms of burnout.

Research shows that when medical care is delivered alongside non-medical services that affect health, patients, providers, and the health care system overall are better off. Social services not traditionally considered medical services, such as transportation and nutrition, are particularly crucial for high-need, high-cost Medicare beneficiaries; in addition to improving health outcomes, they may also lower costs.
