Federally qualified health centers (FQHCs) provide comprehensive primary care and preventive services to more than 31.5 million patients across the United States, regardless of their ability to pay. FQHCs have a unique financing structure that relies on federal grant funding and low Medicaid reimbursements. As a result, they operate on thin, often unpredictable financial margins, which limits their ability to implement and sustain improvements. Value-based payment (VBP) models could help. Under VBP arrangements, payment is tied to the quality, cost, and equity of care rather than the volume of services delivered. Providers are often given an upfront lump sum for each patient, giving them greater flexibility to deliver the right care at the right time. Experts have suggested that VBP could be particularly beneficial for FQHCs in the long run by making funding more predictable, freeing up resources for them to innovate, and supporting greater investment in infrastructure. | | |
Over the past few years, in an effort to promote better outcomes for patients, healthcare providers, payers, researchers, and regulators have started paying more attention to a once-overlooked area of healthcare: chronic wounds. Chronic wounds encompass injuries such as burns and surgical site infections, as well as pressure injuries (bedsores), hospital-acquired pressure injuries (known by the acronym HAPIs), and diabetic ulcers. The advent of value-based care initiatives and heavy penalties for HAPIs are leading to changes in wound care. Not only do these injuries cause pain for the individual, but they also add to a hospital’s costs, are a cause for readmissions, and can lead to additional infections and even death. Dedicated clinicians and innovators bringing promising technologies, products, and services to the market are starting to move the dial in managing costs and improving outcomes. | | |
Optum Health CEO Amar Desai, M.D., took the stage at ViVE on Monday morning to discuss the company’s value-based care ambitions and strategy, but there was a clear elephant in the room: the cyberattack on Change Healthcare that’s still causing disruption nationwide. Desai said the Optum team has been in regular communication with its clients’ technology and cybersecurity leaders as it navigates a response to the lingering challenge. He emphasized that the issue is confined to Change’s systems, and that Optum broadly, UnitedHealthcare and UnitedHealth Group were spared. “We don’t have much more to share than what has been shared publicly, but we continue to work the problem,” he said. “We have external partners also engaged with us in every aspect of it.” | | |
| Andrew Molosky, the president and CEO of Chapters Health System, is keenly aware of the struggles that coincide with turning a large health care provider organization into a value-based one. The first priority in that situation, in his mind, is making sure that the entire organization is on the same page in terms of what value-based care really means, he told Home Health Care News last month at Home Care 100 in Scottsdale, Arizona. “I think the first relationship that you must establish is a pretty good internal harmony with your employee base, your stakeholders, your governance structure, whatever that might be,” he said. “Because nothing will waylay an organization faster than when you have people feeling the priorities differently.” The Tampa, Florida-based Chapters Health System is a community-based nonprofit organization. It provides a range of home-based care services to about 120,000 patients. It has been approaching value-based care as a priority for quite some time now, and with urgency. | | |
