Value-based care encompasses a broad variety of payment models in which health care providers are incentivized to provide better care at lower costs. Eleven years ago, we began presenting information on accountable care organizations (ACOs) including the count of distinct organizations entering into accountable care contracts and the number and distribution of lives covered by these arrangements. ACOs, one model in which providers take responsibility for the overall cost and quality outcomes of a defined population, continue to exist and do remarkable things improving health care delivery, but they are not the only type of value-based care models that exist today. In this article, we will provide an overview of current value-based activity as well as updated ACO data.
A diverse group of mission-aligned health systems, a Medicare Advantage health plan, a healthcare analytics solutions company, the association representing Illinois community health centers, health policy leaders, and other health partners came together today to launch the Midwest Health Equity Coalition. The coalition will serve as a brain trust of experts committed to reducing racial health disparities among underserved seniors in Indiana, Michigan, and Illinois — identifying a set of best practices that can be replicated across the country.
CMS’s recent integration of health equity into its new and existing value-based payment (VBP) programs (as outlined below) signals a strong commitment to equitable care delivery. We expect health equity to be incorporated into additional federal programs and for private payers to follow CMS’s example in designing their own plans for managing the diverse needs of rural, multicultural, high-need, or otherwise historically underinvested populations. Considering these changes, healthcare stakeholders should evaluate their current capabilities for tracking and managing health equity and prepare a strategy to address the social needs of their respective populations. Federal programs often reward early adopters before phasing in mandated policies, so organizations who move proactively can benefit from first-mover advantages and prepare to avoid future penalties.

Type 2 diabetes (T2D) disproportionately affects racial and ethnic minority groups. Black Americans are 50% more likely to have T2D, and are at higher risk for complications, than whites.1 They are also the group with the highest rates of obesity. 40% of Latinos with T2D have not been formally diagnosed, and 45% struggle with obesity. This isn’t just a chronic disease epidemic, it’s a health equity crisis. Many health equity gaps in care may be attributable to social determinants of health (SDOH). SDOH are the social and environmental conditions that people grow up, work, and live in that influence their health, like level of income, quality of housing, access to nutritious food, and experiences of discrimination.
