Primary care physicians (PCPs) and other physicians continue to migrate away from the traditional fee-for-service (FFS) reimbursement model toward value-based and risk-bearing care models that reward better outcomes and reduced healthcare costs. Value-based care (VBC) contracts are projected by McKinsey to cover nearly 65 million Americans, or 22% of insured lives, by 2025, up from 15% in 2021. The trend toward VBC is clear. Yet transitioning from a volume-based transactional billing model to one that incorporates financial risk and rewards tied to the patient’s health outcome is not easy.

Jack Resneck, Jr., MD, a dermatologist from San Rafael, California, was inaugurated here Tuesday as the 177th president of the American Medical Association (AMA). Resneck began his inaugural address by admitting that initially he had had some trepidation about taking the position, given “so much pain and despair in the last 2 years, in the wake of this pandemic and a politically fractured country … Questions about whether we could collectively – all of us in organized medicine – move beyond the serious challenges we face. Now that you’ve made the decision to start your path to Alternative Payment Models (APMs), what’s the first thing you need to consider? Hint: APM revenue calculation does not top the list. Obviously, APM revenue generation vis-à-vis traditional Fee-for-Service is critical. But those calculations assume constancy of two essential “assets”—clinicians and patients.
