Long-term and post-acute care (LTPAC) providers play a vital role in our health system. As one of the most complex patient populations, LTPAC patients are a considerable driver of Medicare costs.

According to the Medicare Payment Advisory Commission, Medicare spent nearly $57 billion on post-acute care—around 15% of total spending. Forty percent of inpatient hospital discharges are followed by LTPAC services, totaling nearly 2 million skilled nursing facility (SNF) stays per year. Continuing to improve and coordinate post-acute care can lead to higher quality and lower costs. As such, LTPAC providers are in a position to lead the way on value-based care. Unfortunately, many existing value-based care models, such as accountable care organizations, do not accommodate the unique challenges these providers face.

Health system C-suites eye value-based care roles

Healthcare is in flux. Hospitals across the nation report clinician shortages and tight margins, and insurers are pushing more care into the lower cost settings.

Forward-looking hospital leaders are feeling the pinch and getting serious about value-based care, which means promoting leaders who can embrace this value transition and innovate for the future. “I anticipate a dedicated set of roles and skills to help navigate the path from fee for service and volume to value-heavy agreements [will be essential],” David Rottinghaus, MD, president of the physician network at Butler (Pa.) Health System, told Becker’s. “We will need to make sure there is a steady flow of revenue to sustain health system operations, while directing resources toward quality arrangements with payers.” But the transition is easier said than done, especially because it can take months for health systems to realize the benefits of hitting quality metrics.

How extending virtual options can drive value-based care

One way Northern Health, a major provider in Melbourne’s north, is creating value for its patients and community, is by enabling its clinicians to offer virtual options. It began during the pandemic. As with the rest of the industry, the organisation too had to accelerate its digital transformation by embracing digital tools for delivering remote care. However, they had to make do with little technologies available to them back then. “Systems were clunky, there was little or limited integration of our systems and poor interoperability,” recalled Dr Katharine See, chief health outcomes officer and director of Respiratory Medicine at Northern Health. Once the dust settled, they realised that they could never go back to the way things were before. Northern Health is now receiving over 114,000 admissions each year; its emergency department alone treats over 111,000 patients annually with the auxiliary Victorian Virtual Emergency Department now attending to calls from more than 103,000 patients.
