While value-based care adoption has accelerated in the healthcare industry, the alternative care delivery and payment model is still lacking in the pediatric setting. The current healthcare infrastructure does not support the resources needed to successfully implement this model for kids, according to Rachel Thornton, MD, vice president and chief health equity officer at Nemours Children’s Health. “So much of pediatric healthcare is about preventing future harms that may require more investment in the short term to achieve those optimal long-term outcomes,” Thornton told RevCycleIntelligence. "That may be one of those places where the current model—the way it’s operationalized from a cost-benefit standpoint or an efficiency standpoint—doesn’t provide the latitude needed to make those early investments in children that ultimately produce the healthier population in the future that is going to cost the healthcare system less.”
Seventeen of the nation’s leading stakeholders in healthcare are backing a bipartisan House bill recently introduced to Congress that would reinforce Medicare’s move to value-based care. The support of the Value in Health Care Act of 2023, a bipartisan legislation that would make several important reforms to ensure alternative payment models (APMs) continue to produce high quality care for the Medicare program and its beneficiaries, was introduced in a letter signed by the stakeholders.
Medicaid expansion is close in North Carolina, with the Republican-led legislature passing and the Democratic Governor Roy Cooper signing the Access to Health Care Options legislation to take effect with passage of the state budget for the 2024 fiscal year. A distinctive feature of this result is the strong bipartisan legislative vote. Other states have expanded Medicaid through the legislative process, but with limited bipartisan support, or have used ballot measures. However, it is likely that action in the remaining non-expansion states will depend on engaging legislators from both Republican and Democratic parties. Here, we describe key contributing factors enabling Medicaid expansion in North Carolina, which could serve as a roadmap for further bipartisan Medicaid reforms. 
Reading between the many lines in the 1,920-page 2024 Medicare Physician Fee Schedule (PFS) Proposed Rule, one thing is clear: CMS is still struggling to move providers into Advanced Alternate Payment Models (APMs) and keep existing ACOs moving forward on the path to value-based payments. The APP Reporting tug-of-war between CMS and ACOs results in a slight concession for providers worried about difficulty and cost of all-patient APP Reporting. We’ve seen this before, of course. Remember the delay in sunsetting the Web Interface for ACOs in the 2022 Rule and the retreat from mandatory transition to risk in the 2023 Rule? This year is no different.
