While medical coding may not be at the forefront of innovation priorities for some healthcare systems, there is real opportunity for care delivery improvement and systemic cost savings. Instead of perceiving coding solely as a necessary mechanism for fee-for-service reimbursement, progressive healthcare leaders are viewing coding as a catalyst for advancement in population health, value-based care and research. Diagnosis (ICD-10) and procedure (CPT) code sets offer substantive data insights to support the patient journey and experience. Clearly, tracking patients’ activities over time is an imperative for clinical decision-making, and clinically specific codes can be an accurate and efficient tool for care providers to summarize relevant and critical information in each patient’s chart. | | |
In 2024, 50 accountable care organizations are new to the Medicare Shared Savings Program (MSSP) and 71 ACOs renewed participation, according to new numbers released by the Centers for Medicare & Medicaid Services (CMS) Monday. The total number of participants now stands at 480 ACO programs, with 245 organizations continuing in ACO REACH and the Kidney Care Choices (KCC), two types of CMS Innovation Center models. More than 634,000 providers and organizations serve nearly 11 million people in traditional Medicare. In 2022, 483 ACOs were participating in MSSP, showing a lack of overall growth in program participation. | | |

When I was 14 years old, my parents moved our family home within Miami Dade County. We shifted from zip code 33157 to 33183, and as I’ve recently learned, this increased our life expectancy from ~79 years to ~86 years. My parents didn’t have a college education or any access to this type of data, but instinctively they moved toward what we all want and should have — safer neighborhoods, good schools, grocery stores with fresh foods, and job opportunities. Zip code determines up to 60% of health, and life expectancy can vary up to 30 years within a 10-mile span. Yet over 50 million Americans live in economically distressed zip codes, and we’re a far cry from all Americans having “the fair and just opportunity for every person to attain their highest level of health,” per the CDC. | | |
Chicago-based Medical Home Network (MHN), a nonprofit organization focused on transforming care in the safety net, is partnering this year with 64 federally qualified health centers (FQHCs) in seven states to participate in two CMS value-based programs: ACO REACH and the Medicare Shared Savings Program (MSSP). MHN provider partners participating in the NeueHealth Premier ACO achieved $10.1 million in gross savings and a 100 percent quality score in the 2022 performance year. This will be the first year MHN participates in the MSSP, and the second year participating in the ACO Realizing Equity, Access and Community Health (REACH) Model. Together, these ACOs will cover nearly 50,000 Medicare lives. MHN supports care transformation by helping FQHCs build whole-person care coordination capabilities at the practice level — powered by AI-driven, real-time analytics and information that gives a full picture of a patient’s health. | | |
